Mass Upload Patients via CSV

Before starting create CSV file with the following columns
External ID | Unique Identifier from EHR |
First Name | Required |
Last Name | Required |
DOB | Required |
Gender | Required |
Email | Contact Email |
Primary Phone | Contact Phone via SMS |
SSN | Encrypted after uploaded |
Street | |
City | |
State | |
zip | Numerical. 5 Digits Max |
Primary Insurance Name | |
Primary Insurance Number | |
Primary Insurance Group ID | |
Secondary Insurance Name | |
Secondary Insurance Number | |
Secondary Insurance Group ID

1. Navigate to Admin.
2. Navigate to Location and select Upload.
3. From the Upload Column select Patient Portal from the dropdown and chose Go.
4. Select the File.
- Click Upload.

Map columns from the CSV to available Patient Fields
5. Click the Finalize button at the top of the dialogue box.

Updates will be reflected immediately
